A 35 year old woman came with loss of vision in the left eye and epistaxis since 20 days
MRI of the maxillofacial region shows a large, lobulated, well-defined expansile lesion centered in the sphenoid sinus. It is hypointense on T1 and T2 images with homogenous enhancement on post contrast images. The lesion Involves both sphenoid sinuses, the left ethmoid air cells and extends into left frontal sinus through the left frontoethmoidal recess, left superior orbital fissure and into the medial extraconal compartment of the left orbit . It displaces the medial rectus laterally. The lesion encases the intra-canalicular and intracranial segments of the left optic nerve and , inferomedially abuts the cavernous segment of left internal carotid artery.
