A two-day-old baby was brought with increasing difficulty in breathing since birth.
The plain film shows mediastinal shift to the right. The left hemithorax has a mix of air and soft tissue shadows. The tip of the nasogastric tube is in the mid body of the stomach, Its proximal side hole is well beyond the gastroesophageal junction. Serial radiographs obtained following instillation of barium in the stomach through the nasogastric tube (with the nasogastric tube in situ) show multiple air and contrast filled bowel loops in the left hemithorax. The left lung is compressed ; the mediastinum is shifted contralaterally. The left dome of the diaphragm is poorly defined.
