A six-year-old girl was brought with pain and swelling around the left elbow and fever since four months
Frontal radiographs of the left humerus show multilayered periosteal reaction in the distal meta-diaphysis of the humerus with thinning and erosion of both the cortices. The medulla is destroyed with a wide zone of transition.MRI study of the distal left humerus shows a large, heterogeneously enhancing soft tissue lesion measuring approximately 3.8x4.2x4.4cm causing cortical erosion in the meta-diaphyseal region. It involves the flexor and extensor compartment. The lesion is Tl isointense, T2 hypointense with restricted diffusion. There is associated lamellated periosteal reaction. There is loss of normal marrow signal in the distal humerus. The neurovascular bundle is displaced eccentrically by the lesion.
