An eight year old girl was brought by her parents with imbalance while walking and involuntary movements of the right upper limb since one year.
MRI of the brain shows a well-defined, intra-axial, solid-cystic mass in the left thalamo-capsular region. The solid component is isointense on T1 and T2 weighted images with heterogeneous post-contrast enhancement. The cystic component follows fluid-signal intensity in all sequences with no post contrast enhancement.
On DTI study, there is anteromedial displacement of the corticospinal tract and anterolateral displacement of the adjacent projection tracts with no destruction.
