A 19 year old man presented with a swelling over the left hand since two years.
On examination there was a warm, tender swelling.
The MRI shows a large multi lobulated PDFS hyperintense, T2 isointense soft tissue lesion in the ulnar aspect of the left hand involving the muscles of the hypothenar eminence. There are multiple dilated and tortuous T2 flow voids in the lesion with edema in subcutaneous tissue. The lesion shows avid post contrast enhancement.
DSA shows multiple feeders from the radial and ulnar arteries supplying the nidus in the the thenar and hypothenar regions. There is an early draining vein into the cephalic venous system.
