A 49 year old man came with complaints of cough and breathlessness since two years and two episodes of haemoptysis over the past one month.
The frontal chest radiograph shows a soft tissue opacity in the left mid and lower zones with shift of mediastinum to the left side. A cavity is seen in the left upper zone. There is a liner area of fibrosis in the right upper zone.
Contrast enhanced CT scan shows fibrocavitary and bronchiectatic changes with near total architectural destruction of the left lung parenchyma. In one of the cavities in the left upper lobe, there is a well- defined soft tissue with the air crescent sign. There is a fibrotic patch with traction bronchiectasis in the right apical segment with loss of lung volume and compensatory hyperinflation.
