A 34 year old man came with complaints of cough and shortness of breath since one month. He has a history of pulmonary tuberculosis.

High resolution CT scan of the chest shows a thick-walled cavity in the right upper lobe. There is an intracavitary soft tissue mass surrounded by a crescent of air (Monod sign) . There are fibrotic and bronchiectatic changes in the adjacent lung.
