A 28 year old man came with the complaint of dyspnea since one month
CT pulmonary angiogram shows a fusiform aneurysmal dilatation of the right branch of the pulmonary artery measuring 61 mm. There is a non-enhancing, eccentric intramural thrombus with calcification with a a maximum thickness of 38 mm, causing 50–60% luminal narrowing. The patent luminal diameter is 30 mm. The thrombus extends into the lobar and segmental branches with normal distal opacification. It compresses the SVC, carina, proximal main bronchi, and the esophagus. PA to Aorta ratio is >1 and RV to LV ratio is >1.
