A 56-year-old man came with a swelling in the left popliteal fossa since four months. He experiences an electric shock-like radiating pain on tapping it.
Ultrasonography of the left popliteal fossa shows a well-defined, round to oval lesion along the course of the tibial nerve superficial to the tibial vessels. The lesion shows a central cystic component with a few calcific echogenic foci. There is a traversing nerve fascicle in this case cystic lesion. Fusiform expansion of the tibial nerve is seen at the site of the lesion.
MRI of the left knee reveals a well circumscribed. ovol, T1 isointense, T2/STIR heterogeneously hyperintense lesion along the course of the tibial nerve, posterior to the tibial plateau. It measures 2.5x2.4x2.1 cm. The lesion shows peripheral thick rind of enhancement witha central non enhancing cystic component. The tibial nerve is seen entering and exiting the lesion: the lesion is relatively eccentric to the nerve.
