A 14 year old girl presented with pain during menstruation and spotting since menarche
Transabdominal ultrasound examination shows a distended left uterine cavity showing internal echoes. A cystic lesion with low-level echoes is seen in the left adnexa, likely representing hematosalpinx or an endometriotic cyst.
MRI of the pelvis shows markedly distended left uterine horn and cervix with hemorrhagic content (hematometra and hematocolpos). An obstructed left hemivagina due to a transverse vaginal septum with left hematosalpinx and a left ovarian endometriotic cyst are seen. The left kidney was not seen
