A nine year old girl was brought with complaints of generalized weakness and low grade fever since 15 days
MRI of the brain reveals a well defined, 7.5x5x5.5 cm sized solid-cystic, T2/FLAIR hyperintense lesion with hypointense center in the right thalamo-capsular region. The lesion enhances peripherally. Crossing the midline, it extends inferomedially into both thalami. It involves the midbrain, both cerebral peduncles, the right hemipons, the right medulla and the superior cerebellar peduncle. It compresses the third ventricle, the posterior horn of the right lateral ventricle and bulges into 4th ventricle with resultant hydrocephalus, periventricular ooze and midline shift of 7.8mm to the left side. There is non enhancing perilesional edema.
