A 21 year old man was brought with fever and giddiness with altered sensorium since three days. At presentation the GCS was E1V1M1
On MRI, there are multiple areas of diffusion restriction in the parietal, frontal and temporal lobes, both cerebellar hemispheres and the frontoparietal deep white matter. There is involvement of the posterior limb of internal capsule, external capsule, fornices on both sides (body, column and crus) and the splenium of corpus callosum. There is diffuse effacement of the sulcal spaces.
Multiple acute lacunar infarcts are noted in the thalami and body of caudate nucleus, occipital region, both the cerebral peduncles, midbrain and pons and cerebellum. There are foci of blooming on SWI associated with these areas of diffusion restriction indicating hemorrhagic transformation.
