A 58 year-old-man came with left upper and lower limb weakness with facial deviation to the right since three months
MR imaging of the brain shows a well defined, circular, intra-axial, supratentorial T2/FLAIR hyperintense, peripherally enhancing lesion with mild perilesional edema in the deep white matter just lateral to the the thalamus. It measures approximately 1.3 x 1.1 x 1.3 cm. MR Spectroscopy reveals increased choline peak.
