A six year old boy with paraparesis since one month was brought by his parents
MRI of the lumbosacral spine and brain shows a long intramedullary T2 heterointense/T1 hypointense, heterogeneously enhancing lesion extending from D12 to S1 levels with expansion of the spinal cord. A T2 isointense lesion with diffusion restriction on DWI and corresponding drop on ADC map with foci of susceptibility on SWI sequence is seen in the interpeduncular and suprasellar regions.
