A 38 year old man came with imbalance while walking and difficulty in speaking. On examination, cerebellar signs were positive with titubations. There was history of head trauma one year ago which had resulted in hemorrhage in the right cerebral peduncle and subarachnoid hemorrhage.
MRI of the brain shows hemosiderin staining and mild atrophy of the right crus of the midbrain. There is T2/FLAIR hyperintensity with bulky appearance of both the olivary nuclei. There is no contrast enhancement.
