On sonography, in the midline, there is a well-defined longitudinally-oriented, elongated, anechoic, cyst-like structure caudal to the conus medullaris. Filamentous nerve roots reach upto it.

The MR oif the spine reveals spina bifida at the level of lower sacral vertebra. A cystic complex is noted in the spinal canal. The cauda equina roots are stretched and attached to this cyst.
