A ten-day old preterm neonate admitted in the NICU, started having intolerance to oral feeds since three days. On examination,the abdomen was distended.

Frontal and lateral radiographs of the abdomen show a large pneumoperitoneum. There is air along the course of the biliary tract suggestive. The bowels loops are normal.
