A 48 year old man presented with the history of sudden onset, severe left side headache with loss of consciousness and fall. On waking, he was found to have weakness in the right upper and lower limbs.
Axial sections of the contrast and non contrast CT scan of the brain show ill defined hypodense regions in the left temporo-parietal region- in the subcortical and deep white matter areas. There are scattered hyperdense shadows in this region with extension into the left lateral ventricle. There is corresponding mass effect including midline shift and compression of the lateral ventricle. The cortical veins in this region are poorly opacified; the left transverse sinus does not fill. A catheter angiogram confirmed these findings.
