FIG 1- Radiograph of the pelvis with both hips shows a soft tissue opacity in the right iliac fossa. There is erosion of the right SI joint and the right ala of the sacrum. The right hip joint is abducted and externally rotated.
FIG 2A,2B,2C- On CT, there is a heterogeneously hypodense lesion arising from the right half of the sacrum with a large exophytic component in the pelvis. There is extension of the soft tissue component into the right sacroiliac joint.
FIG 3A and 3B -There is displacement of the bowel loops and the bladder to the left.
FIG 4A and 4B- T2, T1 sequence images show heterogeneously hyperintense soft tissue lesion arising from the right half of the sacrum. There is an exophytic component in the pelvis. There are multiple cystic areas showing heterogenous T1/T2 hyperintense signal suggestive of blood degradation products.
FIG 5A and 5B- There are few areas of diffusion restriction.
FIG 6- There is extension into the right S1and S2 neural foramina with encasement of the exiting nerve roots.
FIG 7- The lesion shows heterogenous post contrast enhancement.
