Plain and contrast enhanced MR imaging of the brain shows a heterogeneously T2/FLAIR hyperintense, solid lesion with cystic areas arising from the right cerebellar hemisphere. It extends into the right middle cerebellar peduncle and reaches up to the cisterna magna. The lesion shows diffusion restriction on DWI with corresponding loss of signal on ADC. On contrast administration, the lesion shows enhancement. The adjacent sulci are effaced, the pons is displaced and the fourth ventricle is compressed by this lesion. The lateral ventricles are dilated with periventricular ooze.
