A 27 year old man came with weakness in the left upper limb with weakened left hand grip since two and half years
MRI of the cervical spine shows straightening of the lordotic curvature. There is localised thinning and atrophy of cervical cord from the C4 to C6 level. There is a T2 hyperintense intramedullary signal in the anterior spinal cord involving the anterior horn cells from C3-C7 vertebral levels. There are mild posterior disc bulge at C4-C5, CS-C6, and C6-C7 levels causing mild effacement of anterior thecal space without cord compression. There is anterior displacement of dorsal dura on flexion with few prominent flow voids in the posterior epidural space. On flexion, the cervical spinal cord is displaced against posterior vertebral margin with loss of anterior CSF space. There is a crescentic posterior epidural enhancing tissue with widening of the posterior epidural space on flexion.
