A 26 year old man came with weakened grip of the right hand. On examination there was atrophy of the hypothenar muscles.

There is mild atrophy of the spinal cord between C5 and C7. On flexion pf the neck, there is anterior displacement of the posterior wall of the dural sac with prominence of the posterior epidural space (C4-T2) appearing hyperintense on T2W images. There are small, symmetrical, T2 hyperintense signals in the anterior portion of the spinal cord bilaterally.

Hirayama disease is named after Keizo Hirayama, a Japanese physician who first described it in 1959.
