A six-year-old girl was brought with pain and swelling around the left elbow joint with fever since four months
Radiographs of the left humerus show periosteal reaction with lamellations in the distal ometa diaphysis with thinning and erosion of the cortex. There is soft tissue swelling over the distal humerus..
MRI of the distal humerus shows a large, heterogeneously enhancing soft tissue lesion (3.8x4.2x4.4cm) in the distal humerus causing cortical erosion in the meta diaphyseal region. It involves the flexor and extensor compartments. The lesion is Tl isointense, T2 hypointense with restricted diffusion. There is lamellated periosteal reaction. There is loss of normal marrow signal in the distal humerus. The neurovascular bundle is displaced eccentrically.
