A 39 year old man came with left sided chest pain and multiple episodes of vomiting since two days.
Contrast enhanced CT scan of the chest shows a large defect measuring 2.5 x 3.5 cm in the posterolateral aspect of the left hemidiaphragm. There is herniation of small bowel loops and mesentery through the defect into the left hemithorax. The herniated bowel loops are collapsed. The proximal small bowel loops in the abdominal cavity are dilated. The frontal chest radiograph shows a soft tissue opacity in the left lower and mid zones. A few air pockets are seen within this region.
