A 51 year old man came with complaint of pain with in the right shoulder and decreased range of movements since one year. He is know to have rheumatoid arthritis.

There is moderate joint effusion with extensive synovial thickening and multiple, small PDFS intermediate intensity soft tissue nodules in the joint space.
