A 45 year old woman, a non smoker, came with the complaints of chest pain and breathlessness since six months.
The frontal and lateral chest radiographs shows a well defined round to oval soft tissue opacity in the right upper and mid zones with acute angles with the mediastinum suggestive of a lung parenchymal origin of the lesion. There is mediastinal shift with deviation of the trachea anteriorly and medially; the tracheal is also compressed. The CT scan confirms these findings; shows the lesion to be of heterogeneous fluid and soft tissue density in the right upper lobe causing mediastinal shift and severe tracheal compression.
