An 18 year old girl presented with complaints of gradually progressing blurring of vision in the right eye over a period of 2-3 weeks. There was no history of fever. She gave history of occasional paraesthesia in both upper limbs. There was no weakness of limbs or symptoms suggestive of autonomic dysfunction. There was decreased visual acuity in the right eye. Fundus examination showed blurring of the optic disc margins on the right side. Analysis of the CSF showed oligoclonal bands.
Plain and contrast enhanced MRI of the brain, orbits and spinal cord show multiple ovoid, T2/FLAIR hyperintense lesions in both periventricular deep white matter involving the callososeptal surface, a few showing diffusion restriction and post contrast enhancement. The right optic nerve is bulky in its entire extent with post contrast enhancement, T2 hyperintensity and mild surrounding fat stranding suggestive of optic neuritis. There is abnormal T2 hyperintensity with subtle post contrast enhancement in the spinal cord at the C2-C3 and C4-C5 levels.
