A 68 year old woman, who is a known diabetic, presented with the complaints of involuntary choreiform movements of her right upper and lower limbs since one day. Her blood glucose level was 420mg/dl.
The axial T1 weighted image of the MRI of the brain shows hyperintense signal bilaterally in the caudate nuclei and putamina (left more than right). There was no diffusion restriction, susceptibility or post contrast enhancement.
