A 27 year old man presented with fever, cough and shortness of breath since three days.
There was history of pulmonary tuberculosis four years back.
The frontal chest radiograph shows multiple, round, thin-walled cystic lucencies predominantly in both the lower zones (right > left). Some of the cystic spaces show air–fluid levels, giving a “cluster of grapes/ honeycomb” appearance. Prominent bronchovascular markings with tram-track opacities are seen elsewhere in both lung fields. HRCT of the chest shows extensive cystic bronchiectatic changes in both lungs, predominantly in the right upper lobe, right middle lobe, superior segments of both lower lobes and inferior lingula with few of them showing air fluid levels and bronchial wall thickening. There are multiple tiny centrilobular nodules and ground glass opacities in tree-in-bud pattern.
