Contrast enhanced CT scan of the chest reveals a defect in the anterior diaphragm. There is herniation of the abdominal contents through it into the right anterior mediastinum. The contents of the hernial sac include the transverse colon with the hepatic and splenic flexures with mesenteric fat.
